Students Residing On Campus with Medications/Chronic Conditions
All incoming students who are prescribed medications to take on a regular basis are strongly encouraged to plan for medication management visits with their healthcare provider prior to move in. For students with chronic conditions which may require regular or follow up visits, it is advised to make arrangements in advance of arriving on campus. Being proactive by having all appointments scheduled during breaks and days off from school will provide for optimum continuity of care during the school year and prevents problems with obtaining prescription medications. Choose a pharmacy local to campus to have prescriptions refilled during the school year.
If counseling or psychotherapy has been a part of a student’s wellness package, it is strongly recommended families find local providers prior to the start of the semester. A list of local clinics and pharmacies within walking distance of the campus can be found on the Montserrat Health Services website.
Required Health Forms
A completed Immunization Form is required prior to move-in day. All immunizations must be up to date, and the form must be signed by a healthcare provider. Please pay particular attention to tetanus immunization, as this vaccine is recommended every ten years for adults.
A completed TB Risk Form. If you are considered “high risk”, please have required TB testing done prior to the start of school.
A copy (front and back) of your health insurance card (unless you plan on using the college’s health insurance plan)
Optional: Vaccine Exemption Request Form
Health Insurance
Per Massachusetts law, all students enrolled in the college for 9 or more credits MUST be enrolled in a comprehensive health insurance plan. All students are automatically enrolled in the College’s Student Health Insurance Plan, provided by Blue Cross Blue Shield. If you are already covered by a comprehensive plan, you can opt out of the Montserrat plan. The waiver can be accessed at universityhealthplans.com.
If a student has chosen to waive the college’s health insurance, make sure the student’s insurance provides access to healthcare in the Beverly area and that the student keeps the insurance card on hand in the event that medical care is needed during the school year. In addition, please send a copy (front and back) of the student’s health insurance card to [email protected]; this will be kept on file with the Health Services director in case of emergency.
