Documentation Requirements
Montserrat values student’s unique, individual experiences and is deeply committed to identify academic and/or environmental barriers and determine appropriate accommodations. It is the responsibility of any student seeking an accommodation to disclose to the Director of the Academic Access Studio and submit documentation identifying the functional impact of his/her disability and its likely interaction with a particular learning or living environment. Students are strongly encouraged to submit documentation at least two weeks before classes begin.
Documentation Requirements
- The form of a letter or report from a physician, psychologist, psychiatrist, or other medical provider who is qualified to make the diagnosis and is currently treating the student for a disability
- Documentation must be on official letterhead and be signed and dated within the last year
- Include a diagnosis, the functional limitations (impact on academics) and the specific academic accommodation recommendations. Information should also include date and summary of most recent evaluation and the expected duration of the condition (chronic, cyclical, etc.)
Documentation Review
Documentation is reviewed by the Director of the Academic Access Studio to determine whether a disability is covered under the ADAAA, to identify functional limitations and current impact in an educations setting, and identify any reasonable, appropriate accommodations.
